If you need access to Epic applications or documentation for the purpose of supporting an Epic customer through staff augmentation, submit this application for access. Note that Epic will contact the customer to verify your request. As soon as the customer verifies, Epic will begin processing your application.
If you are seeking access to Epic to connect, implement, test, or troubleshoot your software, review open.epic to see the options available to you and fill out the Tech Guidance form if you need additional direction.
Each third-party organization only needs to submit one application per customer, even if you have multiple staff members working with that customer who need Epic access. If your organization's Epic access needs change for a customer, and you need additional Epic access not previously included in an agreement with Epic, submit a new application for access. Epic will review each fully completed application and either send you an executable agreement to permit the customer to give your staff Epic access or notify you of next steps.

Provide the contact information for an authorized executive at your company to serve as the corporate signatory. Should Epic approve your request for access, this individual will receive the drafted agreement for signature. If they are not the appropriate signatory, they may forward to the appropriate person at that organization.

Epic does not grant access to a customer's Epic system for the purposes of connecting, testing, or troubleshooting software developed by your firm. Instead of completing this form, please visit the open.epic Developer Guide for resources on working with Epic as a software developer. Third-Party Organizations and their Subcontractors must submit separate access requests. Independent contractors working through a different third-party organization are considered Subcontractors.
NOTE: Subcontractors are not directly contracted with the Epic Customer.
